We hope that you will find our office to be easily accessible and friendly.  We thank you for entrusting Roy Medical Associates, and specifically Dr. Bakul Roy, with your health care needs.  We look forward to serving you.


Your First Appointment

Your first appointment with Dr. Roy will be an extended visit, typically 30 minutes.  We ask that you check in 15 minutes prior to your scheduled appointment time so that our staff can process your paperwork.  Please note that Dr. Roy's policy is to not perform a physical exam at the first appointment.  If you would like a physical exam, you may schedule that after your first visit.

If it is at least 1 week prior to your appointment, you may mail your registration form to us.  If it is at least 1 business day prior to your appointment, you may FAX your registration to (925) 866-8082.  You may also hand-carry your registration form to your appointment.  Please make sure to review our Office and Financial Policies and Notice of Privacy Practices before signing the registration form.

Please bring the following with you to your appointment:
* Current Insurance Card or Proof of Insurance
* Driver's License
* Copay, if applicable
* Patient Registration Form

Our office space is limited; please refrain from bringing more than one family member with you to your appointment.

We welcome Dr. Kleinman's former patients

Dr. Roy welcomes Dr. Kleinman's former patients to his practice.  If you were previously a patient of Dr. Kleinman, please be assured that while we continue to maintain your medical records from Dr. Kleinman, we are in the process of transitioning those records to a new computer system.  We appreciate your cooperation in completing a new registration form before you first see Dr. Roy, and we would like to thank you in advance for your patience during this transition.

Records Release

If we require records from your former physician, you will be asked to complete a Records Release form at your next visit.
