To obtain medical records, we require a written release, signed by the patient or patient's guardian.  There is a $20 prepayment required for all electronic medical records.  Please complete the Records Release form below and mail it with your payment to:

If you would like to pick up your records in person, you may FAX your request to us.  Please indicate on the form that you will pick up the records.  You will be required to pay the fee when you pick up the records.  Our FAX number for Medical Records is: (925) 866-8082

If you are a former patient of Dr. Kleinman, Dr. Roy is now the custodian of your records.  Paper records (all records prior to October, 2005) are stored off-site.  There is an additional $50 records retrieval fee to obtain off-site records.

Please note that all Medical Records are provided in electronic format on a CD.  Records that are fewer than 20 pages can be faxed to you or your physician if you provide us with the FAX number.  If you do not have access to a computer and would like printed copies of your records, you must specify that you want a paper copy.  We ask that you pick up paper records in person.
