Billing
We are happy to bill your insurance company for you at the time of service. Please provide your insurance card to the front office staff at each appointment. We will coordinate benefits for patients with more than one insurance plan.
If you do not have insurance, we offer a discount to patients who pay in full at the time of service.
Please take time to review our Financial and Office Policies prior to your visit. All patients must sign an acknowledgement that they have read and agree to our policies. Thank you for your cooperation.

Insurance
We are contracted with many major health insurance plans in our area, including HMO's. Insurance companies have many different types of policies. We recommend that you contact the Customer Service number located on your insurance card to inquire as to whether or not we are participating with your particular plan.
Patients are ultimately responsible for payment of all services rendered.
We strongly urge you to know what benefits are covered by your plan. As a courtesy to our patients, we will directly bill your insurance plan for you, including those with which we are not contracted. We do not guarantee coverage for all services rendered, nor does your insurance company. Only your insurance company knows your plan and coverage in detail; if you have questions about coverage, please contact your insurance plan Member Services department.
Co-pays
Co-payments must be paid at the time of service. We accept cash, Visa, and MasterCard.
